Employers must provide the Medical Provider Network notice at the beginning of the program and to all newly hired employees. When an employer changes insurance carriers, a new notice must be given with information about the new carrier’s MPN. Also, every time an employee reports a work injury; they must be given the MPN notice, so it is best to staple the MPN notice to the DWC-1 Claim Form.
